Thank you for asking to register with our practice. We would be grateful if you could fill in the following information so we can start the registration process.
We will also require the history of the pet from their previous vet before we see them unless they are a puppy/kitten. For puppies and kittens please bring any paperwork from the breeder to the initial consult.
We will require an email address if you wish to receive vaccination reminders as we do not send postal reminders anymore.
Please note that there is a £15 administration fee for processing new registrations other than for puppies & kittens.
Once we have the information, we will contact you to arrange your pet’s first check up with us. If your pet is currently healthy there will be no charge for the initial meet and greet consult.
